This form affects your legal rights. By completing and signing this form you acknowledge that you have carefully read its terms and hereby represent and warrant to TRCA that: (1) you are over the age of majority in Ontario; and, (2) you are registering on behalf of a minor and are his/her parent/legal guardian and as such are fully authorized and entitled to enter into this agreement on his/her behalf. In this form, agreement on your own behalf includes agreement on behalf of the Camper.
